OOCYTE DONATION

The age of the oocytes is a major determinant of success in human reproduction. Women are born with a limited pool of eggs that will be used throughout their life. Therefore, the age of the egg will be that of the woman at the time of ovulation. The older the woman is, the older the egg is and the lower the chance of pregnancy.

WHY OOCYTE DONATION?


Assisted reproduction has only one goal: a live birth.
After the age of 42, even in healthy people, eggs have a small or no chance of achieving a full-term pregnancy. Although pregnancy may occur, the risk of pregnancy loss is very high.

Some patients cannot achieve pregnancy with their own eggs, for example:

  • After chemotherapies or radiotherapies that have caused ovarian failure.
  • Due to lack of eggs (early menopause, removal of the ovaries, Turner Syndrome).
  • Lack of response to ovarian stimulation and the impossibility of obtaining oocytes, or multiple failures in IVF with own oocytes.
  • High risk of transmission of genetic disease to their offspring.
  • Advanced oocyte age (43 or older).

All these limitations to achieving pregnancy, are nowadays solved by oocyte donation.

WHAT IS OOCYTE DONATION?


It is in vitro fertilization using donor eggs .
Egg donors are healthy women between 18 and 34 years old. Donation is carried out only if infectious, genetic, ultrasound, hormonal, and psychological testing of the donor are normal. Phenotypic characteristics are used to match recipients, and donors.

WHAT ARE THE EXPECTED RESULTS?


The success rate achieved with egg donation is high (above 50%), as the success rate depends on the age of the eggs and these are all obtained from young women.

WILL EGG DONATION ALWAYS RESULT S IN A HEALTHY CHILD?


This treatment offers a very good success rate, with the SAME risk of genetic complications (Down syndrome, Edwards' syndrome, Patau's syndrome) than that of a spontaneous pregnancy achieved by a woman of the same age as the donor. In other words: if a 45-year-old woman receives oocytes from a 25-year-old donor, the risk of genetic problems is equal to that of a 25-year-old woman, which is considerably low, but not zero. Similarly, the risk of pregnancy loss decreases with the age of the donor (8 to 15%). The risks of MATERNAL pathology (diabetes, hypertension, cesarean section) are those to be expected according to the age of the recipient.

HOW IS IT PERFORMED?


The corresponding donor is studied. If her tests are normal, she is allowed to donate. Matching with the recipient is based on physical similarities.

  • The donor's ovaries are carefully stimulated and multiple eggs are obtained by ultrasound-guided puncture for later fertilization.
  • Previously, the recipient's endometrium (uterine cavity lining) has been prepared (by administering estrogens and progesterone) for hosting the embryo.
  • Once the eggs are obtained, fertilization (ICSI) is performed with the sperm of the recipient's partner or a sperm donor previously assigned.
  • When the embryos are ready, they are transferred to the recipient's uterus.
  • If available, surplus viable embryos are cryopreserved.

Oocyte donation has been widely proven to be an effective treatment with no medical, psychological, or social differences in the offspring compared to spontaneous or IVF pregnancies.
